Community Living Mississauga has been supporting people with intellectual disability since 1955, offering services such as residential supports, employment resources, community engagement, supported independent living, respite and day support. Employment: Permanent, Full-Time

Working under the guidance of a Supervisor or Manager, this position is responsible for individualized service coordination support for people who have an intellectual disability and their families.

Description of Duties

- Provide service coordination support based on goals identified by people supported and their families.
- Make appropriate referrals to services and programs in the community to address the goals identified.
- Keep informed about and utilize the resources available in the community.
- Support people and their families to access assistance during periods of crisis.
- Promote community inclusion, independence and self‑determination for the people supported.
- Work collaboratively with co‑workers, community partners, people supported and their families to solve problems and resolve conflicts.
- Promote the standards set by the organization in terms of Personal Outcome Measures and incorporate these standards when planning with families.
- Identify and report support situations that do not meet the Minimum Standards Guidelines set by the organization.
- Utilize and develop new techniques, options or approaches to support people and their families.
- Apply diversity, equity and inclusion (DEI) principles to support the organization’s commitment to DEI.

Qualifications





- Social Services Worker Diploma (preferred) or Developmental Services Worker Diploma, Child and Youth Worker Diploma with a minimum of 1 year of experience working or volunteering with people who have an intellectual disability.
- Alternatively, a University Degree with a minimum of 1 year of experience working or volunteering with people who have an intellectual disability.
- Valid Ontario Driver’s License (Class G or G2), reliable vehicle and insurance.
- Current valid First Aid/CPR certificate.
- Proficiency in PC computer environment; MS Office (Word, Excel, Outlook, etc.) and other software.
- Excellent interpersonal and communication skills (verbal, non‑verbal, written).
- Ability to work collaboratively in a team and with families.
- Good problem‑solving and conflict resolution skills.
- Experience in Service Coordination, Case Management or similar supports is an asset.
- Understanding of community resources and processes is an asset.
- Bilingualism in English/French or other language is an asset.
