In this part time role, you'll engage directly with clients, planning and evaluating their care, while demonstrating professionalism and accountability. You will also coordinate with nursing supervisors and maintain high documentation standards to ensure compliance with the College of Nurses of Ontario. This role includes morning shifts and bi-weekly weekend rotations to accommodate patient needs.
Key Responsibilities:
• Provide comprehensive care planning and monitoring
• Conduct ongoing assessments for patient interventions




• Notify supervisors of changes in client conditions
• Lead care initiatives with professionalism and judgement
• Attend team meetings as scheduled
Requirements:
• Active registration with the College of Nurses of Ontario
• Diploma in Practical Nursing
• Current First Aid or CPR certifications
• Knowledge of community nursing preferred
• Ability to commit to the on-call rotation
