Key Responsibilities:
• Complete detailed occupational therapy assessments and treatment plans
• Modify plans based on client evaluations and progress
• Promote safety and independence for transitioning clients
• Ensure coordinated care with team members
• Maintain accurate clinical documentation and case records
Requirements:
• Master’s Degree in Occupational Therapy or equivalent
• Registration with the College of Occupational Therapists of Ontario
• Solid communication and collaboration skills
• Ability to work within an interdisciplinary team
• Valid Ontario driver's license and reliable transportation
