Thames Valley Family Health Team is looking for an experienced Registered Pharmacist to support their primary care model. You will work within a dedicated healthcare team that promotes patient-centered approaches while emphasizing teaching and mentorship for learners. Ideal candidates demonstrate expertise in medication therapy and excellent team-oriented skills to optimize patient outcomes.
Key Responsibilities:
• Deliver evidence-based pharmaceutical care and education
• Resolve medication-related issues for patients
• Improve medication profiles within the electronic medical record




• Work collaboratively in a primary care interdisciplinary team
• Participate in quality improvement activities
Requirements:
• Doctor of Pharmacy or Bachelor of Science in Pharmacy required
• Minimum two years of relevant experience preferred
• Registration with the Ontario College of Pharmacists
• Previous experience in a Family Health Team setting
• Ability to maintain professional liability insurance
