About the role:
The Northern York South Simcoe Ontario Health Team is seeking a highly organized, collaborative, and implementation focused leader to support the advancement of the Neighbourhood Health Home model across the NYSS OHT geography. The vision for a Neighbourhood Health Home is an integrated, coordinated network of care providers with collective accountability to provide guaranteed attachment and access to primary care services to every resident of a well-defined geography. The model is intended to strengthen access to team-based primary care, improve attachment pathways for unattached residents, and better connect primary care, interprofessional teams, hospitals, community support services, mental health and addictions, home and community care, public health, paramedicine, and other OHT partners around local population needs. This role will help move the model from planning into implementation by developing practical workflows, coordinating partner engagement, supporting change management, aligning workstreams to OHT priorities and Ontario Health deliverables, and ensuring the model is operationalized in a way that is clear, measurable,
sustainable, and achievable. The successful candidate will be comfortable working in a complex, multi-partner environment where success depends on relationship-building, practical problem-solving, disciplined project management, and the ability to bring structure to emerging work.
What will you be doing:
- Neighbourhood Health Home Implementation
- Support the development and communication of the Neighbourhood Health Home vision
- Support the implementation of the Neighbourhood Health Home model across priority communities, beginning with identified initial geographies and expanding over time.
- Translate the Neighbourhood Health Home vision into a practical implementation plan, including milestones, accountabilities, timelines, risks, and decision points.
- Support the development of hub-and-spoke operating models that connect IPCTs, primary care practices, community providers, hospitals, and OHT partners.
- Develop and document workflows, standard operating procedures, and escalation pathways related to patient attachment, care navigation, referral pathways, and partner integration.
- Help identify which services should be delivered directly through the Neighbourhood Health Home model and which should be accessed through existing OHT partners to avoid duplication.
Alignment with OHT Priorities and TPA Deliverables
- Map Neighbourhood Health Home activities to the NYSS OHT work plan, strategic priorities, and Ontario Health TPA deliverables.
- Ensure implementation activities support key areas such as primary care access and attachment, supported attachment, chronic disease prevention and management, ALC/Home First, cancer screening pathways, care navigation, and population health planning.
- Support the development of clear deliverables, outputs, and performance indicators that demonstrate progress without overcommitting beyond the OHT's direct control.
- Work with OHT leadership to ensure the model is positioned as the operating structure to advance existing priorities, rather than as a separate or competing initiative.
Partner and Primary Care Engagement
- Build and maintain strong relationships with primary care providers, PCN members, hospitals, community providers, and other OHT partners.
- Support engagement with physicians, nurse practitioners, interprofessional primary care teams, family health teams, community health centres, and other primary care partners.
- Coordinate meetings and working sessions with partners to define roles, responsibilities, workflows, and implementation requirements.
- Support the integration of OHT network tables and partner organizations into the Neighbourhood Health Home model in a staged and practical way.
- Help identify opportunities for partners to contribute directly to implementation, including care pathways, navigation supports, chronic disease management, patient education, and community-based services.
