In this part-time role, you will assess the needs of Elders using the RAI-MDS tool and implement care plans based on evaluation outcomes. Your leadership will extend to supervising nursing team efforts and supporting Personal Support Workers throughout daily activities.
Key Responsibilities:
• Administer and document medications and treatments
• Chart care provided and manage Elder inquiries
• Educate staff, Elders, and families on care plans




• Complete RAI-MDS assessments and care plans promptly
• Mentor students during clinical placements
Requirements:
• Registered Practical Nurse Diploma required
• Registration with the College of Nurses of Ontario
• Physical ability to meet job responsibilities
• Knowledge of relevant legislation and county policies
• Willingness to support Elder care and comfort
