As a member of our interprofessional team, you will contribute to a diabetes strategy that emphasizes self-care and the diverse needs of clients. Your duties involve providing footcare, performing health screenings, and ensuring comprehensive documentation for all services rendered. Collaboration with health professionals is crucial to deliver holistic care.
Key Responsibilities:
• Perform footcare and health assessments for diabetic clients
• Advocate for client access to necessary medical supports
• Collaborate with professionals to ensure holistic care
• Plan tailored case management for individual cases
• Accurately maintain medical records following care
Requirements:
• Completion of Advanced Nursing Foot Care course
• Current registration as a Registered Practical Nurse
• Demonstrated cultural competency in healthcare
• Experience in an Indigenous organization is beneficial
• Willingness to travel and possess a valid driver’s license
