Family Transition Place is on the lookout for a compassionate Housing Support Worker to join the HART Hub Dufferin team. In this role, you will be instrumental in delivering comprehensive care and support for individuals facing homelessness and other challenges. You'll work collaboratively with clients, perform assessments, and connect them with resources to secure reliable housing and improve life quality.
Key Responsibilities:
• Educate clients about Housing Support Workers and services
• Complete SPDAT assessments for effective case management




• Engage with clients weekly for individual support
• Coordinate housing advocacy and resource referrals
• Collaborate with landlords to find housing opportunities
Requirements:
• Education in Social Work or equivalent experience
• 2+ years supporting homeless individuals
• Familiarity with Housing First approach
• Excellent written and verbal communication
• Valid driver's license and police clearance required
