Halton Healthcare is looking for a Temporary Patient Flow Navigator to optimize patient movement within our facilities. This critical position requires effective collaboration to address challenges in patient transitions.
In this part time role, you will support operations by coordinating admissions, discharges, and patient flow across multiple sites. You will chair daily meetings with stakeholders while managing bed allocations and tracking performance metrics. Your expertise will contribuit to a responsive care environment focused on patient safety and throughput.
Key Responsibilities:
• Supervise patient admissions and discharge processes
• Lead daily bed meetings with health team members
• Work on optimizing patient placements and throughput
• Assess and respond to patient acuity and needs
• Collaborate on performance metrics tracking
Requirements:
• Minimum 3-5 years in a healthcare leadership role
• Experienced with Meditech and Visibility systems
• Proven record of effective change influence
• Strong organizational skills and critical thinking
• Availability for rotating shifts in a dynamic setting
