Key Responsibilities:
• Develop self-management education for diabetes patients
• Facilitate engagement in group education sessions
• Participate in program planning and evaluation




• Interact with patients across various health center sites
• Collaborate with interdisciplinary health professionals
Requirements:
• Undergraduate Degree in Foods and Nutrition
• Registered with the College of Dietitians of Ontario
• Knowledge of community health initiatives
• Experience in program development is essential
• Proficiency in French is a bonus
