Become a key advocate for First Nation communities as a Community Housing Transition Worker in Winnipeg with the Assembly of Manitoba Chiefs. Your role will focus on securing permanent housing and community resources for clients.
Under the leadership of the Lead Service Coordinator, the Community Housing Transition Worker is tasked with providing advice and advocacy to First Nation citizens seeking reliable housing solutions. You will evaluate client backgrounds and needs, offering tailored support and organizing workshops on crucial social and personal skills. Your interventions will have a direct impact on clients' lives as you help them navigate available community resources.
Key Responsibilities:
• Conduct interviews to gather client information




• Develop personalized action plans for housing success
• Facilitate workshops on life skills and support programs
• Maintain program statistics and client records
• Connect clients with relevant social services
Requirements:
• Diploma in social work or related field
• Experience within social service environments
• Knowledge of trauma-informed practices
• Excellent organizational and communication skills
• Valid Class 5 Driver’s License required
