As a vital member of a multidisciplinary team, the Social Worker will develop and coordinate treatment plans reflecting Patient and Family Centered Care values. Candidates should have a Master's Degree in Social Work and 3–5 years of clinical experience in mental health. You'll engage in community outreach, mental health advocacy, and ensure a healthy patient-centered setting.
Key Responsibilities:
• Provide individual and group therapy sessions
• Conduct psycho-social and risk assessments
• Create and review treatment plans with patients




• Liaise with community agencies for patient support
• Offer crisis interventions and educate staff on therapeutic techniques
Requirements:
• Master’s Degree in Social Work
• 3-5 years clinical experience in mental health
• Eligible for OSWSSW membership
• Knowledge of psychiatric disorders and treatment
