Your tasks include managing resident assessments, service plans, and staffing adjustments, ensuring the highest quality of care. Key Responsibilities:
• Collaborate on resident care assessments and service plans
• Oversee staffing levels based on occupancy and service needs
• Manage Continence Care Program through assessments and orders




• Supervise medication administration and training
• Build relationships with hospital staff for outreach initiatives
Requirements:
• Registered Practical Nurse in Ontario
• Minimum 2 years’ experience in geriatrics or rehabilitation
• Bilingual in English and French preferred
• Strong leadership and supervisory skills
• Proficient in Electronic Health Records and Microsoft Office
