Key Responsibilities:
• Complete MDS assessments and care area assessments efficiently
• Ensure resident documentation correctness for reimbursement

• Monitor and update care plans based on resident evaluations
• Participate in quality improvement initiatives and resident care discussions
Requirements:
• Current LVN/LPN license in your state
• Experience in long-term care settings preferred
• Familiarity with Medicare/Medicaid guidelines and MDS 3.0
