In this pivotal role, you will work closely with community clinicians and Ontario Health to foster competency in palliative care.

You'll lead initiatives aimed at integrating services while providing educational support to healthcare providers. Engaging with Indigenous organizations will also be a key aspect of your responsibilities. Key Responsibilities:
Mentor staff to improve healthcare delivery
Implement local service integration initiatives
Consult on complex patient care issues
Collaborate with Indigenous communities for outreach
Track progress and support data collection efforts Requirements:
Master’s degree in Nursing mandatory
At least five years of relevant practice experience
Registered with the College of Nurses of Ontario
Experience in supervising clinical staff
