Become a key part of Shared Health as an Indigenous Discharge Planning Coordinator focused on patient-centered care. In this vital role, you will work to enhance the discharge planning process for Indigenous patients across Manitoba.
The position emphasizes collaboration with families, care teams, and community resources to provide effective transitions to communities. Your efforts will contribute to a culturally secure setting reflecting Indigenous values and practices. Key Responsibilities:
Support coordinated discharge planning with care teams
Maintain accurate patient records and databases
Facilitate communication with external agencies
Assist in reporting and presentation material preparation
Address inquiries from patients and stakeholders Requirements:
Minimum 2 years with Indigenous patients in acute care
Understanding of Indigenous health challenges and resources
Bachelor’s degree in health or social services discipline
Completion of anti-racism training required
Strong knowledge of Windows-based programs Utilize your expertise in Indigenous healthcare to support Shared Health in Manitoba.
