Make an impact as a Dietitian with Grand River Community Health Centre in Brantford, ON. This role focuses on nutrition care for diverse populations while collaborating with primary care teams. As a Registered Dietitian, you will provide nutritional counseling and develop care plans for clients, particularly those with chronic diseases.
You'll work hand-in-hand with other healthcare providers and community partners to ensure clients receive comprehensive care tailored to their needs. Key Responsibilities:
Deliver individual and group nutritional counseling
Coordinate with interdisciplinary teams for effective care plans
Engage with clients in various community settings
Support education initiatives on cooking and nutrition
Evaluate the impact of nutrition programs on client health Requirements:
Registered Dietitian certification in Ontario
Experience in community health settings preferred
Proven conflict management and interpersonal skills
Familiarity with the social determinants of health
