Your responsibilities include performing comprehensive medication reviews, advising healthcare specialists, and ensuring the safety and effectiveness of drug therapies. This position supports both acute and residential care environments, emphasizing cooperative team efforts. Key Responsibilities:
Conduct medication therapy evaluations and assessments
Serve as the primary drug information resource




Document all findings within patient health systems
Oversee dispensing and monitoring of prescriptions
Communicate recommendations to the multidisciplinary team Requirements:
Registration as a pharmacist in British Columbia
Completion of an Accredited Pharmacy Residency program
Minimum 3 years of clinical pharmacy experience or equivalent
Advanced training in pharmacy practice preferred
