Serve as a Community Connector and enhance youth programs with a Wraparound strategy. Work closely with Children Services to support children and youth in transformative live-in initiatives.
In this pivotal role, you will assist in creating lasting impacts on vulnerable individuals through effective engagement and support. By focusing on strengths-based approaches, you'll work with multidisciplinary teams to establish comprehensive plans that cater to individual needs. Your involvement will ensure the development and sustainability of community relationships that foster independence and wellness for those you serve.
Key Responsibilities:
• Engage with youth to assess strengths and needs
• Strengthen community support systems
• Develop action plans with team collaboration
• Support crisis stabilization and prevention strategies
• Coordinate transitions for individuals in programs
Requirements:
• Diploma/degree in Human Services necessary
• 5+ years experience in similar roles
• Expertise in addictions, mental health, and family support
• Valid first-aid certification needed
• Up-to-date police check and driving license required
