We are currently seeking an experienced and motivated Care Manager, PSW to support and guide our team of Personal Support Workers who provide essential care in the community.

Acclaim Health announces the availability of three (3) Full‑time Care Manager, PSW positions in the Home Care department. These positions are located in the Oakville Office supervising a team of PSWs. The incumbent will work Monday‑Friday, 7.5 hours per day with flexibility for evenings and an on‑call rotation with other team members.

Primary Functions

- Identify, minimize and manage patient risk in accordance with risk‑management and incident‑reporting standards and policies.
- Participate and collaborate with a multidisciplinary care team, including staff, patients and caregivers, in supporting the patient’s needs according to the patient’s care plan.
- Assess and monitor patient status ensuring that collaborative, patient‑centred Care Plans are in place and current.
- Provide internal customer‑service excellence through leading by example, coaching and mentoring to support employees in delivering excellent patient care.
- Under the direction of the Manager, Personal Support, develop and implement quality initiatives aimed at correcting negative variances, addressing gaps, mitigating risks and promoting client safety.
- Collaborate with Human Resources in recruiting and orienting Personal Support Workers.

Effectively manage the performance of direct reports through monitoring and regular performance reviews: manage PSW time‑off (vacation, sick, short‑term LOA), ensure direct reports understand and are aligned to key accountabilities, provide input into the PSW education plan,



lead and co‑facilitate PSW team meetings, and collaborate with the Manager, Personal Support in the annual review and revision of PSW policies.

Qualifications & Education

- Valid Ontario driver’s licence, use of vehicle and insurance per Acclaim Health policy.
- Diploma as a Registered Practical Nurse.

Experience & Skills

- Minimum 2‑3 years as a Registered Practical Nurse.
- Demonstrated ability to coach, mentor and lead staff.
- Strong verbal and written communication and interpersonal skills.
- Effective listening skills.
- Strong assessment, problem‑solving and negotiation skills.
- Strong planning, coordinating and organizing skills.
- Knowledge and skills in quality/risk management.
- Proven ability to multi‑task and meet deadlines.
- Proficient in Microsoft Word, Excel, Outlook and Alayacare.
- Knowledge of community resources.
- Basic knowledge of Acclaim Health and its services.
- Ability to work independently and collaboratively as a team member.

Additional Information
