SOAHAC is hiring a full time Indigenous Patient Navigator to enhance the health journey of Indigenous individuals. Responsibilities include advocating for Indigenous wellness within healthcare settings and ensuring culturally protected access to services. Your role will promote understanding of treatment options and support effective communication between patients and healthcare providers.
Key Responsibilities:
• Provide navigation support for Indigenous healthcare access
• Build trusting, culturally relevant relationships with clients
• Educate clients on healthcare options and discharge planning
• Advocate for culturally appropriate care within Western systems
• Facilitate connections to Traditional Healers and community supports
Requirements:
• Solid preference for Indigenous background and community connection
• Relevant credentials in nursing, social work, or human services
• 3–5 years' experience with Indigenous communities and healthcare
• Knowledge of systemic health barriers facing Indigenous people
• Excellent skills in advocacy, communication, and relationship building
