In accordance with the established mission, vision, values, and strategic priorities of Northern Health, and under the general supervision of Manager or designate, patient safety is a priority and a responsibility shared by everyone at NHA, and as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position. The Intake Coordinator assists in Intake Service for Specialized Services by coordinating and triaging all referrals,



liaising with community providers, providing resources and contacts as necessary and monitoring the intake service and patient flow on a regular and ongoing basis. Provides direction and support to referrals sources on referral pathways and procedures. Liaises with Interprofessional teams to identify opportunities for Specialized Services program involvement. Provides limited direct clinical service to patients, families and groups including family assessment, and psycho education.
