Key Responsibilities:
• Offer personalized support to promote housing stability
• Conduct regular client check-ins and wellness assessments

Requirements:
• Diploma or Degree in Social Services or related field
• Minimum 3 years in Mental Health or Substance Use support
• CRM technology knowledge preferred
• Excellent problem-solving and interpersonal skills
• Valid First Aid/CPR certification
