You will play a significant role in facilitating discharge processes that honor Indigenous traditions and values. Partnering with families and care teams, you will help ensure a trauma-informed and culturally secure workplace for Indigenous patients in Manitoba. Your commitment to patient-centered care will be crucial in fostering effective communication and coordination.
Key Responsibilities: • Address inquiries from patients, families,
and stakeholders • Maintain accurate records related to patient discharge • Collaborate with care teams on patient transitions • Support referral processes and communication with agencies • Assist with data reporting and informational materials
Requirements: • Minimum 2 years working with Indigenous populations • Knowledge of health systems affecting Indigenous communities • Bachelor’s degree in a relevant field • Experience with anti-racism and cultural safety training • Proficiency in Microsoft Office suite
