VHA Home HealthCare is expanding its Palliative Care program and looking for dedicated RNs and RPNs. In this role, you will assess and coordinate care for clients while collaborating with a multidisciplinary team to enhance the quality of life for individuals and their families. You will also manage your schedule and enjoy the flexibility of home health nursing.
Key Responsibilities:
• Assess and plan patient care effectively
• Monitor and evaluate care interventions continuously




• Ensure high-quality patient care delivery
• Maintain accurate client documentation
• Participate in a standby/on-call rotation
Requirements:
• RN or RPN registration in Ontario
• Valid CPR or First Aid certification
• Experience in acute care and community nursing
• Robust communication and documentation skills
• Valid Ontario driver’s license and reliable vehicle
