The Caseworker in Treatment Foster Care is a pivotal member of the team, managing a caseload and providing essential clinical leadership and resources. In this role, the Caseworker is responsible for conducting clinical assessments, offering counseling, developing treatment plans, advocating for clients, and coordinating team training and case management. They also offer vital clinical consultation to treatment foster parents, reunification families, and educational teams, ensuring seamless communication between all stakeholders through written reports, consultations, and daily interactions.
Key Responsibilities
Services
- Develop individualized and family-centered treatment plans in collaboration with stakeholders.
- Provide clinical oversight and lead the implementation of treatment plans,
including documentation and communication with stakeholders.
- Conduct clinical assessments and refer participants for specialized services (psychological assessments, counselling, clinical resource needs, adjunct therapies, and psychiatric services) as needed.
- Communicate with referral agencies and attend clinical appointments with participants when required.
- Provide individual, group, and family counseling services.
- Advocate on behalf of participants.
- Lead planning conferences, family consultations, and meetings with healthcare professionals.
- Participate in educational conferences and meetings with other stakeholders as needed.
- Serve as a liaison between treatment foster families, education programs, and other stakeholders.
- Influence the therapeutic environment of the family home by offering team training and education sessions.
- Provide post-crisis support and direct clinical follow-up when appropriate.
- Complete reports and document all casework clearly, accurately, and promptly.
- Disseminate significant information to families and referring agencies.
