Humber River Health is hiring a Social Worker for our Outpatient Mental Health and Addictions Program to deliver vital community-based services. This full time temporary role focuses on improving patient care and outcomes in North West Toronto.
As a Social Worker, you will assess mental health needs, provide counseling, and deliver group education to patients. Your commitment to holistic care and teamwork will enhance the rehabilitation process in our energetic outpatient program.
Key Responsibilities:
• Complete detailed mental health assessments and risk evaluations
• Provide counseling to individuals, families, and groups
• Conduct education sessions addressing mental health issues




• Collaborate on discharge planning and community resource referrals
• Ensure accurate documentation of patient interactions
Requirements:
• Master's degree in Social Work from an accredited school
• At least two years of experience in relevant settings
• Certification in foundational CBT principles
• Understanding of motivational interviewing techniques
• Excellent communication skills and adaptability to change
