ISTC seeks an enthusiastic Community Psychiatric Nurse to provide intensive home care for older individuals with severe mental health problems. This full time position involves working collaboratively in Calderdale’s mental health facilities.
In this Band 5 role, you will provide comprehensive assessment and treatment as part of a dedicated multidisciplinary team. Delivering care in the community is fundamental to improving patient outcomes and experiences. You will also engage with service users, ensuring their needs are met through planned interventions.
Key Responsibilities:
• Conduct home-based treatments in place of hospital care
• Engage and collaborate with service users and families
• Ensure compliance with medication administration procedures




• Manage referrals and support service enhancements
• Engage in ongoing career development activities
Requirements:
• Registered Mental Health Nurse or equivalent
• Experience with acute mental health situations
• Capability to work flexible schedule, including on-call
• Strong teamwork and independent working skills
• Access to a vehicle and valid driver's license
