Key Responsibilities:
• Assess and identify patient health and wellness risks
• Coordinate management of complex chronic conditions




• Teach self-management strategies to patients
• Document patient care in Electronic Medical Records

Requirements:
• Bachelor’s degree in nursing
• Licensed with the College of Registered Nurses of Alberta
• Minimum 2 years of nursing experience preferred
• Proficiency in motivational interviewing techniques
• CPR certification required
