In this temporary part-time role, you'll deliver patient-centered care, enhancing recovery and independence for older adults. Your responsibilities include comprehensive assessments, nursing interventions, and collaboration with an interdisciplinary care team. You will be crucial in preparing clients for discharge while ensuring their medical and psychosocial needs are met.
Key Responsibilities:
• Utilize clinical knowledge for comprehensive patient assessments
• Plan, deliver and evaluate nursing interventions effectively
• Provide client-centered care addressing personal and spiritual needs
• Act as a preceptor for nursing students and peers
• Support smooth transitions to alternative care settings
Requirements:
• Valid registration with the College of Nurses of Ontario
• Current BCLS Level C and AED certificate
• Bachelor of Science in Nursing (BScN)
• Experience in psychogeriatrics is preferred
• Robust interpersonal and problem-solving skills
