Purpose: The Assertive Community Treatment Team (ACTT) services clients aged 18 and older with serious mental health challenges and significant functional impairments. ACT is delivered by a group of multidisciplinary mental health practitioners who work as a team and provide the majority of the treatment, rehabilitation, and support services to achieve their goals directly in the clients environment of choice.

Duties: Collaborate and contribute to a client-centered treatment plan.

Provide assessments, consultation, and clinical treatment within the context of a multi-disciplinary team.

Demonstrate competence working in an independent and flexible manner in a community-based setting.

Supervise, develop, revise and maintain pharmacological and medical treatment in collaboration with the team psychiatrist.

Provide educational support to community partners and other hospital staff on psychiatric issues.

Provide supportive visits, assessments, and treatments in the community and client homes Effectively use the Standards of Nursing Practice and the nursing process to assess, plan, implement, and evaluate client care.

Provide health teaching to clients Comply with legislation, and organizational and departmental policies, standards, and procedures.

Maintain commitment to self-growth and career development by remaining current in knowledge and application of best practice through the utilization of appropriate research and literature.

Participate in ongoing quality improvement and risk management activities.

Other duties as assigned.

Qualifications:



Current certificate of competency in Ontario Valid drivers license and access to a dependable vehicle Experience: Minimum of two years psychiatric or addictions medicine nursing experience required Skills: Demonstrate good interpersonal and conflict resolution skills.

Knowledge of the Mental Health Act, Suicide Intervention, Health Care Consent Act, and Personal Health Information Protection Act Strong problem solving and critical thinking skills.

Knowledge and understanding of the standards of practice of the College of Nurses of Ontario Demonstrated knowledge of antipsychotic medications and their side effects Demonstrated knowledge in the recognition of mental health diagnoses included but not restricted to schizophrenia, anxiety disorders, mood disorders, personality disorders, and substance use Ability: Ability to work independently in the community Demonstrated ability to liaise with referral sources, community agencies and other team members to enhance positive client outcomes Demonstrated ability to maintain successful working relationships; ability to meet and interact with people in a pleasant, professional, responsible and reassuring manner, both in person and on the telephone; tact, courtesy and alertness in public contacts.

Demonstrated verbal and written communication skills Physical ability to sit for long periods of time and climb stairs Knowledge and understanding of the hospitals mission statement and core values.
