This part-time (0.6 FTE) Registered Nurse position is essential to the operations at the Two Rivers Family Health Team, collaborating with healthcare providers to optimize patient outcomes. You will take on key nursing duties while actively engaging in health promotion and chronic disease management across outpatient services.
Key Responsibilities:
• Provide comprehensive nursing care following College of Nurses standards
• Facilitate effective communication within the health care team
• Assess and triage patient needs during consultations




• Document visits and care in Electronic Medical Records (EMR)
• Support disease prevention strategies and patient education
Requirements:
• Licensed Registered Nurse with the College of Nurses of Ontario
• 2-3 years of relevant clinical experience
• Knowledge of primary care is an asset
• Solid assessment and triage skills
• Excellent team collaboration and communication abilities
