Working under the Manager of Discharge Planning, you will support patients and families in timely hospital transitions. Your expertise in critical thinking and psychosocial assessments will be essential for complex discharge scenarios. Collaborating with an interdisciplinary team, you’ll address safety gaps and implement discharge plans effectively.
Key Responsibilities:
• Facilitate complex discharge planning for diverse patients
• Conduct in-depth assessments and create collaborative plans
• Identify and resolve safety challenges with appropriate resources




• Assess need for psychosocial interventions and services
• Ensure patient flow through effective communication
Requirements:
• Bachelor's in Social Work, Nursing, or Health Discipline
• 2-5 years of clinical experience in acute care
• Essential experience in discharge planning
• Knowledge of community resources and legislation
• Excellent collaboration and problem-solving skills
