With a strong background in case management spanning four years, applicants should also possess a degree in health or human services, like RPN or RN. Key Responsibilities:
• Develop and coordinate client assessment applications
• Facilitate accurate referrals to long-term care facilities
• Maintain and update waitlists for eligible clients




• Engage with clients and families for effective placement
• Complete essential background checks for compliance
Requirements:
• 4+ years in long-term care or community care environments
• Extensive knowledge of service options and case management
• Relevant health degree or diploma required
• Exceptional interpersonal and negotiating skills
• Possession of a valid driver’s license and personal vehicle
