Position: Aurora-Newmarket Family Health Team (ANFHT) Social Worker
Qualifications: Master of Social Work degree and experience in the mental health field, ideally both adults and seniors
Reporting to: ANFHT Executive Director

JOB SUMMARY:
The Social Worker will play an integral role in the care of the patients of the Aurora-Newmarket Family Health Team by assisting the ANFHT in achieving its vision and mission.
The role of the Social Worker in the Family Health Team requires a thorough knowledge of primary care within the scope of the MSW class and the ability to function effectively in a team environment with physicians, other integrated health professionals and the administrative team.
The role also requires a high level of interpersonal skills to deal with patients and their families in a holistic compassionate fashion.
Education and Qualifications:
· Master of Social Work from a recognized University
· Current membership and in good standing with the Ontario College of Social Workers and Social Service Workers of Ontario
· Working knowledge of the Mental Health Act, Healthcare Consent Act, Substitute Decisions Act, other relevant legislation as well as community mental health and addictions resources
· Experience working as a Social Worker, MSW working with patients and families providing counseling
· Demonstrated knowledge and experience with a variety of crisis assessment and intervention techniques and support strategies for mental health and/or addictions
· Efficient computer skills, working knowledge of electronic medical records
· Experience in a multidisciplinary setting will be considered an asset
Roles & Responsibilities:
Assessment
· Receive referrals from physicians and integrated health professionals (IHP) relating to patients with possible
issues pertaining to mental health
· Conduct preliminary sessions where appropriate and determine next steps
· Detailed assessment of mental health issues relating to individual and family well-being
· Write up initial findings and consult with ANFHT clinician(s) to determine recommended plan of treatment
· Field calls from persons in distress and direct appropriately; follow up to determine outcome
Treatment/Management
· Provide counseling, education and support to patient, family and other care givers
· Provision of treatment through individual, couple, family and group counseling and psychotherapy to achieve optimum social and psycho-social functioning
· Case management, including links to community services
· Specifically for seniors, provide counseling for specific issues including adjustment to disability/illness, palliative care, intergenerational/family conflict, abuse, social isolation
Education/Advocacy
· Participate with other team members in the provision of group programs and learning
· Identify and support access to community,
volunteer and professional services and programs appropriate to the needs of the patient
· Provide patient education and individual and group sessions to patients in the understanding of particular health issues
· Assistance with system navigation and coordination
· Advocacy to access resources and supports
Referrals/Collaboration
· Collaboration with other team members to optimize patient health
· Provide communication to hospital and community staff and other interdisciplinary team members to ensure smooth transition for care
· Referrals to community services, as appropriate
· Community outreach and linkages
