JOB SUMMARY

Under the direction of the Clinical Manager Healthy Living, the Community Health Nurse is responsible for delivery of Community and Public health programs within the organization to support clients in community to access services both on and off the nation.

JOB DESCRIPTION

The Community Health Nurse will work collaboratively with health professionals, elders, youth, and community members to ensure they are receiving the utmost care. The role includes daily work with clients within all programs at Stoney Trail Wellness Center (STWC). Your role will be to assist with immunizations, medication administration, wound care, case management, and other medical needs. Education, mentoring, and community training are key components of the position. The nurse will be expected to make informed decisions that contribute to the overall safety and wellbeing of the community.

DUTIES AND RESPONSIBILITIES

Nursing Duties

· Report to the Clinical Manager Healthy Living daily

· Planning, implementing, and delivery of Community and Public Health initiatives

· Delivery of Immunization Program in accordance with Indigenous Service Canada policies

· Support Maternal Health program with immunizations, newborn assessments, and education

· Providing health education and awareness to community

· Provide screening clinics, education, awareness (individual/group) for community on chronic disease, diabetes, sexually transmitted disease

· Assess, monitor, and follow up with the holistic needs of chronically ill and refer to Home and Community Care if necessary

· Provide Footcare Services where applicable and/or necessary

· Connect with Elders and Knowledge Keepers in community to support the traditional ways of healing

· Case manages all clients within program and updates team lead of any changes

· Build relationships and trust within the community to support clients with services, referrals, and additional support





· Utilize nursing knowledge and skills to provide preventative, therapeutic, and rehabilitative care

· Perform independent nursing functions using established procedures, policies, protocols, and licensing standards

· Deliver patient care tasks safely and efficiently

· Collect data and statistics to support case management, care planning, and client goal setting

· Maintain communication between clients, significant others, healthcare team, and partners

· Carry out physician and nurse practitioner orders accurately

· Evaluate nursing care effectiveness and adjust plans when needed

· Document observations and interventions accurately in all charting systems

· Administer medications and monitor/document client responses

· Perform administration and documentation of prescribed injections

· Provide thorough shift reports with relevant client information

· Report client needs, changes in condition, and updates to the circle of care

· Conduct community and home visits daily for medication administration and follow-up

· Collaborate with the health team, contracted services, and on/off-reserve partners

· Mentor, educate, and train junior staff as required

· Support nursing team with additional duties and client care

· Maintain ongoing education and training to ensure secure and effective practice

· Apply critical thinking and respond appropriately to urgent or emergent situations

· Maintain continuous communication with off-reserve services to ensure high-quality care

· Promote and model culturally competent care practices

EDUCATION, SKILLS & QUALIFICATIONS





· Licensed Practical Nurse (LPN) or Registered Nurse (RN)

· First Aid and CPR

· Experience working with vulnerable populations

· Transport of Dangerous Goods (preferred)

· Immunization Certification

· Foot Care Training (preferred)

· Mental Health First Aid (preferred)

· Trauma-Informed Care (preferred)

· Cultural awareness and competency (preferred)

· Understanding of cultural uniqueness within Treaty Seven Territory is an asset

· Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint)

· Excellent verbal, written, and presentation skills

· Strong leadership abilities and abilities to mentor and supervise community members, youth, elders, and health staff

· Strong interpersonal, listening, and coaching skills

· Excellent project management and organizational skills

· Must be friendly, courteous, assertive, objective, and fair

EMPLOYMENT CONDITIONS

· Clear Criminal Record Check and Vulnerable Sector Check

· Valid Driver’s License

WORKING CONDITIONS

· Community-based position with both office-based work and daily home visits

· Must be comfortable driving in winter road conditions, with potential wildlife encounters, including dawn/dusk driving

· Hours per Shift: 7 hours

· Shift Pattern: Monday through Friday
