Yaletown House Society is a non-profit long-term care home in the heart of Vancouver dedicated to providing exceptional, person-centred care for older adults with complex care needs. We are seeking a compassionate, collaborative, and experienced Occupational Therapist to join our interdisciplinary team.

In this role, you will work alongside a dedicated rehabilitation team that includes a full-time Rehabilitation Assistant and a part-time Physiotherapist. Together, you will play an integral role in helping residents maximize their independence, maintain functional abilities, and enhance their quality of life through evidence-based, resident-centred rehabilitation services.

What You'll Do

As our Occupational Therapist, you will:

- Complete comprehensive occupational therapy assessments for residents upon admission and throughout their stay.
- Develop, implement, and evaluate individualized treatment care plans using evidence-based practice.
- Assess mobility, functional abilities, cognition, psychosocial functioning, seating, positioning, adaptive equipment, and activities of daily living.
- Acts as a resource to interdisciplinary team, residents, families and other disciplines.
- Supports families by working with 3rd party vendors to acquire specialized equipment to meet the resident’s care needs.
- Recommend and implement interventions that maximize resident independence, safety, and quality of life.
- Collaborate closely with physicians, nursing staff, physiotherapists, rehabilitation assistants, residents, and families.




- Participate in interdisciplinary care conferences and family meetings.
- Provide education and consultation to staff, residents, and families.
- Complete required documentation and RAI-MDS assessments accurately and within established timelines.
- Support quality improvement initiatives and contribute to the ongoing development of occupational therapy services.

Qualifications

We're looking for someone who has:

- A Bachelor's or Master's Degree in Occupational Therapy from an accredited program.
- Current registration in valuable standing with the College of Health and Care Professionals of British Columbia (CHCPBC).
- Eligibility for membership with the Canadian Association of Occupational Therapists (CAOT).
- Minimum two (2) years of recent clinical Occupational Therapy experience, preferably in long-term care, geriatrics, rehabilitation, or a healthcare setting.
- Strong clinical assessment, problem-solving, and communication skills.
- A passion for working with older adults and providing compassionate, resident-centred care.
- The ability to work independently while contributing positively within an interdisciplinary team.

Skills

The ideal candidate will have:

- Strong clinical assessment and evidence-based treatment planning skills.
- Experience working with older adults in long-term care, rehabilitation, or a healthcare setting.
- Knowledge of mobility, activities of daily living, adaptive equipment, and falls prevention.
- Excellent communication, organization, and documentation skills.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Strong clinical judgment, problem-solving skills, and a commitment to person-centred care.
- Proficiency with electronic health records and computer applications.

Pay: $45.16-$56.17 per hour
