POSITION DESCRIPTION
Assess needs and support women, families, and community members (45%)
• Connect with and build strong relationships with women and families who are referred to or in need of more

intensive support.

- With a trauma-informed lens, supporting women and families to navigate government and community

programs, advocating on their behalf where required for successful outcomes.

- Support women and families to move into housing. This includes helping them arrange utility hook up,

financial planning, food planning, locating furnishings, community mapping, and developing housing
preservation plans to reduce the risk of future homelessness.

- Liaison with landlords to build positive relationships and understand available housing options.
- Build and maintain positive relationships with other YWCA departments to support women and families in

accessing others YWCA services and programs that meet their needs.

- Provide warm referrals to community agencies and programs in support of individual needs and activities.
- Maintain productive programming relationships with external partners that support the community in

accessing food security, housing needs, employment / income opportunities etc.

Deliver effective case planning and management to support women and families (45%):
• Follow common case planning process with each individual as outlined by the Outreach Case Management

team, including intake/assessment process and development of person- centered case plans, goal setting, and
regular home visits.

- Conduct follow-up assessments and regu l ar check-in as needed.
- Commitment to up-to-date community programm ing and resources ava il ab le and supporting access.
- Maintain detailed and accurate electronic case files aligned with internal processes and funder needs and

expectations.

- Complete 3-6-9-12 month SPDAT assessments and track results in HIFI.
- Track referrals, contacts, and other stats in accordance with program standards.
- Attend weekly/monthly meetings (in-house/CAR) to coordinate caseload and ensure regular communication.

Connect women and families to cultural supports, programming, and ceremony (10%)
• Actively work towards devel oping familiarity with Indigenous cultural traditions and engagement with

Indigenous people and communities.

- Access and support women and families in accessing and participating in Indigenous cultural and ceremonial

programming and content.

Education
• Bachelor's degree in social work, Justice Studies, or other human service fields, OR an equivalent
combination of education and experience

Experience

- Demonstrated experience working with the complex needs of people experiencing homelessness
- 2-3 years of demonstrated experience in the field of human service delivery
- Experience working with Indigenous communities and cultures




- 2-3 years of case management experience is preferred

Knowledge, Skills & Abilities

- Knowledge of the Child Welfare system, Trauma Info rm ed Care, and Fam ily Violence would be an asset
- Strong critical thinking skills and ability to work independently and with a team;
- Strong record-keeping ability and basic computer skills
- Effective communication and organizational abilities
- Crisis intervention skills
- Cultural awareness and respect for women and children of various backgrounds

ASSETS & PREFERRED QUALIFICATIONS

- Knowledge of HIFIS

CONDITIONS & PHYSICAL REQUIREMENTS

- Work Location: Primarily community-based, with approximately 75% of time spent in the community and

25% based out of the office at the Kikaskitânaw Centre, 12th Avenue, Regina, SK.

- Physical Demands: Standing, walking, and some lifting of furniture and other goods.
- Schedule: 8-hour shifts based on a 37.5-hour work week; both day and evening shifts are available.
- Pre-Employment: A criminal record check, including a vulnerable sector check where applicable, may be

required following a conditional offer of employment. Results will be assessed in relation to the duties of the
role and will not automatically disqualify candidates.

- Licenses & Certifications: A valid Class 5 driver's license and current First Aid/CPR certification are

required.
