Case Manager

Position Type: Full-time, temporary—six-month term
Schedule: Monday to Friday, daytime hours
Rate of Pay: To be determined

Position Summary

The Edmonton Bone and Joint Centre (EBJC) is seeking an experienced Registered Nurse to join our multidisciplinary team as a Case Manager.

Reporting to the Clinic Manager | Team Leader and Executive Director, the Case Manager coordinates and supports patients throughout their surgical and non-surgical care journeys. Acting as a central point of contact, the Case Manager works closely with surgeons, patients, families, caregivers and healthcare providers to promote safe, timely and patient-centred care before and after surgery.

Key Responsibilities

- Coordinate multidisciplinary patient care across the continuum.
- Work directly with surgeons to identify and plan patient-care requirements.
- Coordinate and support surgeon clinics.
- Provide patients, families and caregivers with education about their care pathways.
- Develop individualized care plans for surgical and non-surgical patients.
- Communicate effectively with patients, families, team members and external healthcare providers.
- Coordinate appointments, testing and services with internal and external stakeholders.
- Review patient charts for accuracy, completeness and timely ordering of required tests.
- Ensure patients are medically prepared for surgery and that all required testing has been completed.
- Document patient information accurately and promptly in the Electronic Medical Record (EMR).
- Collaborate with Medical Office Assistants to manage waitlists and prioritize patients appropriately.
- Respond to patient telephone inquiries within 24 hours.
- Advocate for patients and help address barriers to care.
- Protect the confidentiality and security of patient and organizational information.
- Participate in quality-improvement initiatives, staff meetings and ad hoc committees.
- Perform other related duties as assigned by management.

Qualifications

- Graduate of an approved professional nursing program.
- Current registration and active practice permit with the College of Registered Nurses of Alberta (CRNA).
- Current Basic Life Support/CPR certification for healthcare providers.
- Minimum of five years of recent clinical nursing experience.
- Demonstrated experience coordinating patient care across the healthcare continuum.
- Experience with program or service development, implementation and evaluation.
- Experience working within a multidisciplinary healthcare team.
- Orthopaedic, surgical, perioperative or case-management experience is considered an asset.

- Alberta Health Services and other health authorities
- Alberta Bone and Joint Health Institute

Job Types: Full-time, Temporary
Contract length: 6 months

Pay: $43.22-$55.94 per hour
