The Clinical Coordinator works closely with the Clinical Manager, Ambulatory Care to promote and facilitate excellence in the DCRP and DEP. Specifically, assist with coordination of diabetes activity including community programs and education, transition planning for pediatric patients, reporting accountabilities and supporting day to day coordination of clinical services overall. Responsible for ensuring effective communication with other program components, staff scheduling, patient appointment coordination and facilitation of workflow, new staff hiring and training, as well as specific educational and training programs for Unit staff.

The clinical coordinator is responsible for promoting and fostering an environment that encourages clinical excellence, continuous learning and improvement, evaluation, research and client-centred care. In doing so, the Clinical Coordinator will work in collaboration with the inter-qualified care team and community partners, while also promoting new relationships to further the mission of the program.

The Clinical Coordinator will be responsible for: assisting in facilitating the development of the inter-professional care team and quality improvement activities; maintaining and leveraging databases, review and reporting for funding contracts, assisting with development,



implementation and evaluation of clinical pathways and/or new program designs.

The Clinical

University Baccalaureate

Degree in the Health Sciences –Master’s Degree preferred

- Minimum 2 years’ experience in applying advanced diabetes knowledge, physical assessment skills, diagnostic interventions and self-care management skills for patients in an ambulatory and inpatient setting

Educator (CDE) and asset 3. Demonstrated excellence in leadership, communication, organization, team building, conflict resolution
