This individual will hear the customers voice and assist them in achieving their health needs through the development of strong relationships and individualized holistic care plans. Practitioners, Primary Health Care Case Manager, Home Care, Emergency Medical

Services, Pharmacy, Therapy Services, Public Health, Dieticians, Psychiatrists,

Psychologists, Social Workers, Addiction Workers, and visiting specialists as this position requires networking with other professionals and agencies within the community. No Experience Experience w/ clinical interventions in Integrated Care or Community Health Care Experience with clinical interventions related to Primary Health Care Experience in brief treatment modalities in PHC setting is an asset Education Degree in Human Services (Social Work, Psychology, Registered Nurse) Competencies Excellent documentation skills Communication skills Critical thinking skills Knowledge and Abilities Ability to evaluate the mind-body-behavior connection Demonstrates ability to accept and implement change Knowledge of principles and implementation of behavioral health skills Demonstrates empathy, trust and respect in the provision of care Knowledge of Health Information Protection Act and other legislation.

Ability to support improved functioning.

Ability to work autonomously with high levels of responsibility & accountability Ability to work flexible hours Skilled in Motivational Interviewing and mindfulness practices Ability to work independently and as a member of a multi-disciplinary team Ability to work under stress Mental Health Act, Child and Family Services Act, Health Care Directive Adherence to the Code of Ethics.
