JOB PURPOSE The Human Service Counselor assesses, supports and assists clients / patients within the community or hospital-based setting through the implementation of therapeutic and rehabilitative programs.

QUALIFICATIONS

- High school graduation plus an additional human services program/course of over six months and up to one year or equivalent.
- For community based positions in Additions and Mental Health must have 1 year of related experience.
- Must posses a valid license.
- Ability to attend work on a regular basis.
- Ability to perform the duties of the position.
- Good work record.

KEY RESPONSIBILITIES The Human Service Counselor is responsible for:

- Participating in the assessment, development and implementation of the client support services plan.
- Observing, documenting and reporting, physical, behavioral and psychological changes in the client.
- Reviewing, evaluating and modifying individual service plans as part of the multidisciplinary team.
- Intervening in crisis situations and providing emotional support to clients.
- Assisting clients with activities of daily living by teaching and counseling on lifestyle skills such as parenting, financial planning.




- Transporting and accompanying clients to activities and/or appointments related to their care.
- Organizing and implementing individual and group therapeutic programs
- Assisting clients to access services of various government and community agencies.
- Showing others how to perform tasks or duties as part of the departmental orientation.
