Become a valued team member as a salaried GP at Two Rivers Medical Centre, working 4 to 6 sessions weekly. Provide comprehensive care to a diverse patient population within a collaborative environment. In this role, you will execute essential GP responsibilities including triaging, conducting consultations, and home visits.

You will assess healthcare needs, manage disease risks, and educate patients on health issues. A focus on developing tailored patient care plans in accordance with current disease management protocols is also critical. Key Responsibilities:

- Conduct face-to-face and phone consultations efficiently
- Handle queries and medical paperwork accurately
- Assess patient healthcare needs and risks
- Implement care plans based on established protocols
- Maintain detailed consultation records for audits

Requirements:

- Fully licensed GP with GMC registration
- On the NHS England medical performers list
- Proficient in chronic disease management
- Committed to continual learning and development
- Excellent communication skills with patients
