Lead impactful psychiatric care at WelbeHealth as a Psychiatric Mental Health Nurse Practitioner. Balance direct patient interaction with pivotal clinical leadership roles in a community-focused setting. In this position, you will address the needs of a smaller patient caseload, fostering comprehensive relationships to promote high-quality mental health care.
Collaborate closely with interdisciplinary teams and physicians to devise and implement individualized care plans for elderly participants. Your leadership will directly influence the success of our behavioral health programs. Key Responsibilities:
- Deliver psychiatric care and collaborate on participant evaluations
- Mentor and lead the behavioral health clinician team
- Engage in strategic discharge planning with IDT
- Supervise nursing home visits to optimize patient outcomes
- Shape best practices through clinical audits and workflows Requirements:
- Master’s degree in Nursing with a psychiatric emphasis
- Certified Psychiatric Mental Health Nurse Practitioner
- At least four years working with elderly populations
- Valid state nursing license, along with CDS, NPI, and DEA
