Job Description

nMission: As an independent contractor, or direct hire, the Nurse Practitioner will provide comprehensive primary care to residents as part of the interdisciplinary health care team.n

Key Responsibilities

n

- Provides comprehensive primary care to residents.
n
- Is knowledgeable of the Resident Bill of Rights and endeavors to respect and promote it.
n
- Conducts person-centered health assessment/physical of residents on admission with specified time frame.
n
- Comply with applicable provincial legislation pertaining to long‑term care, Long‑Term Care Homes Act 2007 & Regulation 79/10.
n
- Participates in regular focused resident and family care conferences.
n
- Manages the care of residents by providing pharmacological, complementary and/or counseling interventions.
n
- Provides outreach and transitional services to LTCH residents who return to the community.
n
- Collaborates and consults with physicians, director of nursing and personal care nursing staff, interdisciplinary team and external resources regarding resident plan of care.
n
- Provides leadership and involves the interdisciplinary team in quality improvement initiatives.
n
- Provides formal and informal teaching and coaching in the management of clinical care.
n
- Supports Clinical team with programing.
n

Qualifications

nn
- Master’s of Nursing or equivalent with a clinical focus.
n
- Completion of a Post‑Master of Nursing (NP Field) Diploma.
n
- Registration with the College of Nurses of Ontario,



preferably within the RN (Extended Class).
n
- Prefer 3–5 years of clinical nursing experience, preferably in Long‑term Care.
n
- Innovative critical thinking with excellent communication, public speaking and problem‑solving skills.
n
- Knowledge of theoretical frameworks and research application.
n
- Seeks out opportunities to maintain competency and continued learning.
n
- Maturity and good physical and mental health, empathy, patience and tact are important qualities to possess.
n
- Liability Insurance, WSIB Clearance Certificate.
n
- Current (within 6 months) Vulnerable Position/Sector Screening (VPS).
n
- TB Screening Complete.
n
