Key Responsibilities:
• Provide health assessments and therapeutic interventions
• Collaborate with an interdisciplinary team for patient care
• Lead community health education programs and evaluations
• Manage health promotion and illness prevention activities
• Conduct necessary referrals and follow-ups for patients
Requirements:
• Bachelor’s degree in Nursing essential
• Must hold Extended Class RN status via CNO
• Preferred Master's degree in Nursing
• Experience in primary care or community health
• CPR and First Aid certifications required
