Position Overview
We are seeking a dedicated and dynamic Client Care Coordinator to join our team. In this role, you will be responsible for overseeing client care services, conducting consultations, supporting caregivers, and ensuring the highest level of client satisfaction.
This position is ideal for someone who thrives in a fast-paced environment, has strong interpersonal skills, and is passionate about making a difference in the lives of seniors.
Key Responsibilities
Client Care & Relationship Management
- Conduct in-home consultations with prospective clients and families
- Assess client needs and develop personalized care plans
- Facilitate client and caregiver introductions to ensure smooth onboarding
- Build and maintain robust relationships with clients and their families
- Ensure ongoing client satisfaction and retention
- Identify opportunities to enhance or adjust service plans through regular follow-ups to better meet clients' evolving needs.
Quality Assurance & Monitoring
- Conduct regular quality assurance visits and check-ins
- Evaluate and update care plans based on client needs and feedback
- Address and resolve client concerns or service issues promptly
- Ensure all services meet company standards and compliance requirements
Caregiver Support & Coordination
- Collaborate with caregivers to ensure proper delivery of care
- Participate in caregiver performance discussions and coaching when needed
- Foster a positive, supportive, and team-oriented work environment
Operations & Documentation
- Maintain accurate and up-to-date client care plan and records in the system
- Coordinate care schedules in collaboration with the operations team
- Participate in team meetings and contribute to continuous improvement
- Ensure confidentiality and professionalism in all interactions
- Maintain clear, open, and effective communication with the Director of Care and team members.
Qualifications
- Nursing designation (LPN or RN) is required
- Experience in home care, senior care, or case management is required
- Robust communication, problem-solving, and organizational skills
- Ability to work independently and as part of a team
- Valid driver's license and access to a reliable vehicle
- Proficient in basic computer systems and documentation
Work Schedule
- Full-time: Monday to Friday in-office position in NW Calgary
- Travel to client homes for consultation and check-ins
