This regular full-time role focuses on supporting clients accessing integrated care pathways for substance use disorders. Collaborating closely with an interdisciplinary care team, you will manage a caseload, coordinate services, and develop discharge plans. Your efforts aim to create a stable and supportive environment for those facing mental health and substance use challenges.
Key Responsibilities:
• Develop detailed discharge plans with client-specific goals • Maintain community support through appointment accompaniments • Build partnerships with external service providers • Manage active caseload of 15-20 clients • Participate in team meetings and case evaluations
Requirements: • BA/BSW with 2 years in human services • Experience with clients facing mental health and substance use issues • Knowledge of harm reduction principles • Solid skills in crisis intervention and conflict resolution • Proficient with MS Office and data management
