The Ontario Shores Centre for Mental Health Sciences is seeking a Registered Practical Nurse to join the Forensic Dual Diagnosis Transitional Housing Rehabilitation Program (DDTRHP) within the Forensic Outpatient Service (FOS). Reporting to the Clinical Manager, the successful candidate will be a member of an inter-professional team providing specialized services to individuals with a dual diagnosis who are under the jurisdiction of the Ontario Review Board and living in the community.

Within the DDTRHP program, the Registered Practical Nurse will support the coordination of community-based care aligned with recovery-oriented principles grounded in compassion, inspiration, and hope. Working from a bio-psycho-social framework, the nurse will collaborate with inpatient teams to coordinate referrals to the program, gather referral documentation, and participate in case conferences for prospective clients.

Working in partnership with the Behavioural Analyst, the nurse will support the behavioural support plans and assist community partners in the effective application of these. Following a client’s transition to the community, the nurse will monitor behavioural and risk management plans, support adherence to Ontario Review Board dispositions, contribute to the development of ORB reports, and provide discipline-specific nursing services.

The Registered Practical Nurse will also play a key role in coordinating access to community resources and providing consultation, education, and support to community partners involved in client care,



including staff from Karis (the residential service provider).

The successful candidate will be a Registered Practical Nurse with strong clinical knowledge in mental health and addictions. Clients within the program live with severe and persistent mental illness and require specialized tertiary mental health services. The nurse will work collaboratively with clients and families, the multidisciplinary team, referral sources, and community partners to support successful, recovery-oriented transitions to community living.

This role is primarily community-based and located off-site, with the majority of the workday spent at a satellite community partner location.

Requirements:

- Registration with the College of Nurses of Ontario in valuable standing.
• Current CPR – Health Care Provider
• It is highly encouraged that staff complete the RPN Mental Health certification within five years of hire date
• Demonstrated knowledge of College of Nurses of Ontario (CNO) Standards of Practice and commitment to maintaining reflective practice standards as described by the CNO.

- Demonstrated knowledge of Forensic Mental Health and ORB is an asset.




• Demonstrated knowledge of Canadian Standards for Psychiatric-Mental Health Nursing, as well as relevant policies and legislation, such as the Mental Health Act and Personal Health Information Protection Act (PHIPA).
• Proven ability to work effectively as part of a multidisciplinary team.
• Demonstrated competence within the full scope of practice of a Registered Practical Nurse in the provision of assessment, care, medications, interventions and documentation of care needs.
• Implements and models the Recovery philosophy in all aspects of care as a Recovery champion

- Ability to assess risk in the community.

- Knowledge of community resources
• Proven excellent interpersonal skills and effective oral/written communication skills.
• Meditech experience preferred
• Demonstrated excellent organizational, problem-solving, decision-making, conflict resolution and critical thinking abilities.
• Ability to perform the physical demands of the position.
• Proven good attendance record with the capability of maintaining this same standard.
• Recent psychiatric nursing and/or forensic experience in a mental health facility or service preferred.
• The ability to work with clients of diverse ethno-racial and cultural backgrounds and proficiency in a second language is an asset.
