Summary of Duties

- Collaborate with the Nephrology inter‑skilled team to support patients with advanced chronic kidney disease (CKD) in modality selection and kidney transplant planning.
- Lead coordination of transplant assessment processes, including identification of eligible patients, arrangement of required testing/consults, timely referral and follow‑up of results.
- Provide comprehensive patient and caregiver education on dialysis modalities (home and in‑centre), kidney transplantation, and conservative care to support informed, shared decision‑making.
- Conduct focused nursing assessments, monitor disease progression, and contribute to individualized care planning for patients approaching renal replacement therapy.
- Promote home dialysis therapies and support timely access to appropriate treatment modalities aligned with patient goals and clinical needs.
- Coordinate referrals for dialysis access and collaborate with the Nephrology Access Coordinator and care teams to support timely treatment initiation.
- Provide cross‑coverage for Nephrology Nurse Clinicians as required to ensure continuity of care.
- Facilitate seamless transitions between MCKC, dialysis programs, and transplant services, ensuring effective communication of patient status, goals, and care plans.
- Act as a key resource for transplant coordination and modality planning, supporting patients and families through complex care pathways.




- Participate in quality improvement initiatives and support program priorities related to home dialysis uptake, transplant access, and optimal care delivery.
- Contribute to data collection and tracking related to transplant progression to support reporting requirements.
- Provide mentorship and education to nursing colleagues and inter‑professional team members related to CKD management, dialysis modalities, and transplantation.
- Utilize electronic medical records and other systems to support documentation, communication, and coordination of care.

Qualifications / Skills

- Current Ontario College of Nurses Certificate of Competence
- Member of RNAO
- BScN degree required
- CNeph(C) required
- Minimum of 5 years recent experience working with Nephrology patient population required with clinical expertise related to all dialysis modalities and transplant
- Knowledge of computers including MS Office Suite applications and related Nephrology software
- Recent adult education program development and teaching experience
- Advanced knowledge of consultation process as related to individuals, groups and/or systems
- Ability to integrate best practice guidelines into hospital policy and procedures
- Demonstrated leadership, organizational, process improvement and project coordination skills required
- Demonstrated communication/interpersonal, collaboration, teamwork, problem solving and decision making skills required
